The Case Manager, Enhanced Follow-up Support Services is part of Urban Health and Homelessness Services (UHHS) and is responsible for providing specialized enhanced follow-up supports to adults and seniors exiting homelessness. Working from a trauma informed, client centered, harm reduction approach, case managers provide one-to-one supports, home visits and telephone contact to assist participants in identifying and achieving self determined goals. Working closely with participants and external service providers, the case manager liaises with and coordinates access to services that assist with housing stabilization and community integration.

This position will support the objectives of the National LIP Secretariat (NLS), which is a joint project of the Toronto South Local Immigration Partnership (TSLIP) and Jasper LIP designed to improve the efficacy of LIPs across Canada through building sustainable networks, shared resources, capacity building, and the amplification of diverse LIP voices. Activities of the NLS include a combination of information sharing, policy analysis and input, communications, capacity building support and serving as a collective voice for LIPs at national decision-making tables. The successful applicant for this position will lead NLS equity and anti-racism initiatives and serve as a resource person/expert for LIPs across Canada seeking to further intersectional equity work in their communities, including LIP efforts to foster newcomer engagement initiatives. They will promote community mobilization on social issues, as well as support community-based, action-oriented research on issues related to anti-racism, equity, and indigenous reconciliation and their impact on immigrants. Other responsibilities include analyzing public policy as it affects newcomers to Canada, facilitating meetings, as well as planning, designing, implementing, evaluating and coordinating professional development workshops and tools for LIPs across Canada.

The Corner Drop-in is a busy, dynamic program that serves ~ 200 participants each day. We provide daytime shelter, nutritious hot food, primary health care services from on-site nurses, doctors and psychiatrists, information and help finding affordable housing, mental health support, voluntary financial trusteeship, substance use counselling, workshops and activities. Relief staff will work as a member of the Drop-In team to provide an enriched drop-in setting for homeless and socially isolated adults. Working alongside staff, volunteers and peer workers, relief staff will ensure a safe and welcoming environment. This position works on an on-call, as-needed basis to replace door and/or floor staff in drop in. Relief staff will help maintain order and safety, participate in crisis intervention, assist drop-in members with accessing services, and make referrals.

The position provides mobile, rapid eviction prevention services to vulnerable households across greater Toronto as referred by the City’s Central Referral Centre. The EPIC worker provides assessments, referrals, creates a client centered action plan and provides other supports as needed. In consultation with the EPIC team, the EPIC worker will liaise with landlords, coordinate mediation and conflict resolution as appropriate.
If an eviction is prevented, the EPIC worker will provide short-term housing stabilization supports. In the event an eviction occurs, the EPIC worker will work with the Housing Help program to re-house and stabilize the clients in their new home. This position works as part of the Corner Drop-in inter-disciplinary team and works to provide an enriched drop-in service for homeless and socially isolated adults.

The Manager TDIN reports directly to the Vice President of Urban Health and Homelessness Services (Vice President) and has a broad range of internal (e.g., governance, staffing etc.) and external (e.g., advocacy, managing key relationships etc.) duties and responsibilities. This is a hands-on role whereby the incumbent works closely with TDIN member agencies who are dedicated to building a socially-just Toronto which is safe, healthy, and inclusive. Member agencies range in size from small individual drop-in programs to larger drop-ins located in multi-service agencies. The Manager’s role is to facilitate access to resources to the over 49-member agencies across Toronto and ensure access to needed supports that enable each drop-in to continue to focus on their joint mission and vision.

As a member of the UHHS department, this position provides direct case management supports to adults living in Covid-19 shelter recovery/isolation hotels as well as clients living in different shelter and respites around the GTA. This support will serve clients with a complete range of complex issues, including health, financial, housing, legal and life skills issues. As an MSSP Shift Lead, you will play a pivotal role in coordinating day-to-day shift activities and operations. Leveraging your own personal experience with homelessness, mental health challenges, and substance use, you will serve as a guiding force, assisting, inspiring, and mentoring both clients and fellow team members on their journeys toward well-being. Utilizing a trauma-informed approach, you will empower individuals to navigate and access services, de-escalate challenging situations, share valuable resources and information, build trust and rapport, facilitate communication with clients, offer social and recreational support, organize and lead client-focused programs as needed, and advocate on behalf of clients when necessary. The MSSP Shift Lead will also accompany clients to appointments. Furthermore, the MSSP Shift Lead will undertake responsibilities such as welcoming and training new team members and collaborating with other teams, including the MSSP Supervisor(s) and the Senior Manager of Mobile Community Health Services. Effective communication between TNG and site operators is crucial to ensure that the needs of our clients are met efficiently. Additionally, you will actively contribute to overdose prevention and response, safety planning, and the enhancement of harm reduction practices through various strategies and advocacy efforts. The ideal candidate for this position will embody the philosophies of our Agency and Peer Program, adhering diligently to all policies, procedures, and requirements.
